CMA is grateful to the Jeffrey P. Ossen Family Foundation for renewing its grant to continue our Community Outreach & Education Project (COEP). COEP is part of CMA’s ongoing collaboration with multi-disciplinary organizations to better serve people in need. Learning from one another and our clients, we develop systemic solutions at the local, state and federal levels. With support from the Ossen Foundation, CMA created and leads an Advisory Team of service organizations and government officials that work together to enhance knowledge about key programs, resources, skills, and ideas for positive change. Together we work to advance access to home and community-based programs and care through outreach, education, and advocacy. We act locally and think globally to improve life for all people, especially people with chronic conditions, their families, and their communities.
